*Medical certificates must be:

  • Provided by practitioner who is not related to you and with whom you have a professional doctor-patient relationship
  • Provide sufficient information to make a determination of the impact of your circumstances on your ability to complete the assessment
  • Received by the assessment team no later than five working days after the assessment.
  • Be dated:
    • For circumstances known in advance, no more than three months prior to submitting this application 
    • For circumstances occurring immediately before or during the assessment, no later than two working days after the date of the assessment
