Primary Health Tasmania has created a COVID-19 guide for aged care facilities.

In addition to the guide and to better enable communication between general practitioners and aged care facilities, a form has been designed to ensure facilities have access to relevant detailed contact information for GPs  in the event of an outbreak. To limit the number of times you may be asked for this information, Primary Health Tasmania will collate this information and provide each aged care facility with a list of their visiting GPs and other relevant information.  It will also be used to help identify if there are any potential gaps in general practice services to aged care facilities in the event of an outbreak.

If you wish to participate in this initiative, please complete the first four questions on the form even if you do not have any patients in a residential aged care facility – this will assist in avoiding duplication and any unnecessary contact.
