If you are preparing for your CBD assessment or thinking of enrolling, here are some important tips to consider: 

  • It can take six months from enrolment, to complete your CBD assessment 
  • Case notes and supporting documents must be submitted in original form. Do not alter, edit any text or hand write your own cases  
  • Recency of cases is important; the last consult must be within three months of the date of submission 
  • Thoroughly de-identify case notes, including supporting documents.  

The assessment team often picks up identifying information including lab, collection, order, and accession numbers. 

Use the approved redaction options such as Adobe black overlay redact tool OR manually printing and redacting with whiteout. 

Important information to keep, includes: 

  • Patient DOB and age 
  • Relevant supporting documents - on their pages with original headers, not included in body of the consult notes.
